Witam,

jestem totalnym laikiem i nie potrafię znaleźć błędu w tym formularzu
byłby ktoś skłonny rzucić na to okiem?
(sprawdzałem serwer na innym skrypcie tego typu (html + php) i PHP działa prawidłowo)
